ProIV Unlimited (“ProIV,” “we,” “us,” or “our”) operates this website, online store, and related administrative and customer-support services (collectively, the “Services”). This Privacy Policy explains how we collect, use, disclose, and retain personal information when you visit proivunlimited.com, create an account, purchase a product or service, request care, communicate with us, or otherwise use the Services.

Shopify hosts our online store and processes information needed to provide the ecommerce portions of the Services. Licensed healthcare professionals, medical groups, pharmacies, laboratories, and other clinical partners may also collect information under their own privacy notices and, when applicable, Notices of Privacy Practices. This Privacy Policy applies to information ProIV handles in its own capacity and does not replace a clinical provider’s or pharmacy’s separate notice.

Personal Information We Collect

Depending on how you interact with us, we may collect the following categories of personal information:

  • Identifiers and contact information: name, email address, telephone number, billing and shipping addresses, account identifiers, and similar information.
  • Account information: login credentials, preferences, account settings, and records associated with your account.

  • Shipping carriers and fulfillment providers that deliver eligible products;

Health Information

Information you provide for care coordination or medical clearance may be sensitive. We use and disclose it only as reasonably necessary to respond to your request, coordinate with the relevant licensed professional or service partner, comply with law, protect safety, or as otherwise authorized by you. Information maintained by a covered healthcare provider, medical group, pharmacy, or laboratory may be subject to HIPAA or other health-privacy laws and that party’s separate privacy notice.

Do not use ordinary email, text message, or the general contact form to send information you consider highly sensitive unless specifically instructed to do so through an approved secure channel.

Data Retention and Security

We retain personal information for as long as reasonably necessary for the purposes described in this Policy, including to provide the Services, maintain transaction and care-coordination records, comply with legal and accounting obligations, resolve disputes, and enforce agreements. Children and Minors

The online store and account features are not directed to children. We do not knowingly collect personal information from children under 13.
